Position Overview

The Home Health Aide Trainee role is an entry-level position within Aveanna’s Pennsylvania operations. Once certified, work settings may include homes, schools, or daycare facilities, depending on the plan of care. Applicants or current employees selected for this position will enroll in Care Academy, a structured training program designed to help individuals earn their Home Health Aide Certification.

Job Duties And Expectations
  • Participate fully in the Care Academy Training Program, including regular attendance at in-person sessions, active engagement, completion of all assignments, self-study materials, and meeting program certification requirements.
  • Communicate effectively with branch leaders, team members, and Care Academy Training staff.
  • Uphold strict confidentiality regarding patient, family, and client-related matters during and after the training program.
  • Demonstrate a commitment to high-quality customer service and a willingness to learn.
  • Aveanna reserves the right to remove trainees from the Care Academy program for any compliance or policy violations or insufficient participation.
Job Duties After HHA Certification
  • Upon successful completion of the Care Academy program, trainees will transition to the Home Health Aide role, receive an updated pay rate, and be assigned patient cases accordingly.
  • Ongoing education will be required post-certification, including meeting Pennsylvania state requirements:
    • Completion of 75 hours of training, including 16 hours of clinical instruction.
    • At least 12 hours of in-service training annually, which may occur during patient care. Aides must demonstrate proficiency in clinical assessments, documentation, and adherence to care policies and procedures.
Requirements
  • High school diploma or GED.
  • Valid driver’s license and acceptable motor vehicle record (only required for approved patient transport; not applicable for pediatric patients in Pennsylvania).

Responsibilities

In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.

Qualifications
  • Be currently licensed as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience is preferred
  • Demonstrate knowledge and skill in current nursing practice
  • Possess a valid state driver’s license and automobile liability insurance

Caregiver must be active at the time of bonus payout and be at a minimum of an 85% electronic visit verification to receive the sign-on bonus. Bonuses will be paid within 30 days of reaching eligibility. New caregivers as well as previous caregivers who have not worked for Help at Home since prior to 1/1/2024 will be eligible for the bonus. Caregivers transferring from an organization within the Help at Home, LLC portfolio of companies are not eligible to receive the bonus.

Requirements:

Qualifications:

  • Education: Master’s degree in Occupational Therapy from an accredited program.
  • Licensure: Current Texas licensure as an Occupational Therapist Registered (OTR) and registration with the American Occupational Therapy Association (AOTA).
  • Preferred: One year of experience in pediatric or home health occupational therapy.

Responsibilities

Our Physical Therapists (PTs) examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. Our Physical Therapists (PTs) use our Specialty Programs, including Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program to treat our patients.

Qualifications

Physical Therapists (PTs) must meet the following requirements:

  • Be currently licensed as a Physical Therapist (PT) in the state of employment
  • A minimum of one year of clinical experience is preferred
  • Demonstrate knowledge and skill in current therapy practice
  • Possess a valid state driver’s license and automobile liability insurance

We are looking for a compassionate and skilled Nurse in the Espanola/Santa Cruz, NM area , to administer patient care in accordance with a physician established care plan. This role offers a low acuity caseload of up to 42 hours per week. We’re seeking RNs to support our growing need for low-acuity patient care across central and northern New Mexico. This opportunity is ideal for nurses who want consistent hours with the flexibility to manage their own schedules.

This role is a 1099 contract opportunity, with care being provided at the patient’s residence. Patient needs range from weekly hours to 24-hour care. Patients range from high acuity to those who only need basic medical management. This opportunity offers flexibility in the scheduling.

Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.

You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNAs, HHAs, Case Managers, and the patient’s physicians.

Offers are contingent upon passing a background check.

Responsibilities
  • Execute physician prescribed plans of care
  • Complete compliant documentation of the care provided in the system of record
  • Manage and administer prescribed medication, treatment, and therapies
  • Conduct patient assessments, coordination of care
  • Perform various activities associated with daily living
  • Monitor vitals and intake/output
  • Assess skin integrity and administer wound care
  • Monitor for changes in the patient’s condition such as weight changes, self-care abilities, and indicators of disease progression
  • Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients
  • Provide education and training to the patient, their family, and/or the Home Health Aide
  • Work one-on-one in the patient’s home to provide a comfortable environment
  • Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants
  • Perform other personal care services as necessary to meet the patient’s needs
Minimum Qualifications
  • Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing
  • Current State or Compact License as a Registered Nurse or Licensed Practical Nurse
  • Current CPR certification
  • TB test and physical examination
  • Maintain required insurance
  • Good interpersonal skills
  • Hand-eye coordination and manual dexterity
  • Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients
  • Ability to operate under stressful conditions and make quick decisions
  • Must have dependable transportation and be willing to commute to the patient’s home
  • Ability to provide basic medical management up to medically fragile end of life care
Preferred Qualifications
  • 1+ years of experience as an RN/LPN in a clinical or home health setting
  • Ability to assist with two person assist

The Occupational Therapist in Home Health is responsible for assessing and evaluating patients' care needs related to functionality, activities of daily living, fine motor skills, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions.
